Jake Tyler Dison, age 18 days, died on Monday May 3, 2010 at Huntsville Hospital. A graveside service will be on Wednesday May 5, 2010 at 10 am at Tri Cities Memorial Gardens.
Jake was the infant son of Kellie Sizemore and Josh Dison.
